Im no expert only been keeping koi for around a 1 year. But as geejay says looks like swim blader problem, it could be a virus or bacterium or it could have food compactions ( constipated ). Do you feed dry food?, if so i would suggest soaking them for a few mins before feeding so most of the air in the food expands when soaking up the water. A friend of mine had this problem and he fed his fish frozen peas (thaw them out first) supposed to clear any blockage, you could also try fasting your fish for three or four days as if there is compacted food this may return things to normal. Also check your water parameters as you should do every week. Hope this help you out good luck with the fish, let us know how it gets on.
