Steve, any idea what actually causes it ?

yes freddy of course
beni is pigmentation , some koi have it on the surface only , and not deep into the scale and skin . on these fish beni is very unstable , and can be enhanced by spirulina and other enhancers , but when it decides to fail , thats it ...it's gone , think of beni as either emulsion , or a good paint job with primer and sealer .
the fish with good beni , where its deep , and strong are often light to medium orange when young, and if you bend them you cant see any white between the scales .
but be aware that many males can have that deep red beni , and it still be stable , from a very young age , just do the bend test !!
not to sound to harsh , but fish with good beni and other characteristics , are never cheap !!
this is now where you have to decide whether you want a pretty cheap koi or something that will grow and improve !!
remember everything we do , is to either keep , enhance or stabilize beni .... beni is a pain in the arse !!!
