Any suggestions on the treatment would be kindly appreciated. All parameters are fine.

It looks like a bacterial infection to me, are its scales starting to lift just behind its gill or is it just the photo? if they are it is probably dropsy, do you have a hospital/quarentine tank?

it looks like a bactirial infection on the koi .
if you can get it into some salt in a tank half a ounce per gallon for a few weeks and it should clear it up and feed it some antibiotic food .
if you need any food for it call me and i can make some up for you and send it .

it is dropsy for sure try what darren said and try feeding propopilis sprayed on the food ,that is presuming he is still eating ,.your vet can inject this koi and although it looks like it may be a little late it is probably its best chance.it should cost about £30-40 at a vets but they get the anti biotics to where the fish needs it ,Combat is another good anti bacteriacide.Good luck it is heartbreaking to lose your koi , but dropsy is very hard to beat.as darren said get it out of the pond away from your others first of all.
Sorry I was in a rush today, but hopefully what was said should make the koi + the others rapidly improve in condition. Pop in to see me tomorrow and I'll sort you out with the stuff you need.
The suggestion this fish needs anitbiotics is a bit harsh dont you think? This is the exact reason all the effective treatments are almost banned! Overuse of treatments mean they dont work when you really need them.
Thanks to all for their responses. The treatment will be starting today and will keep you updated. A massive thanks to Chris at Kitsu koi for his time and advice. Was very interesting actually being able to see what is on a koi under the microscope. May have to look for one now. Thanks again and will see you at the show Chris.
i dont think the use of anti biotics in this case is extreme at all , when else would you use them? this koi,s obvious bacteria infection ( dropsy ) , could only benefit from an injection.What would you consider a case for anti biotics if not this? This koi is indeed in trouble.I know antibiotics are not always the way forward , i just dont understand that when a koi is this badly infected with bacteria ,how it couldnt help? Its only my opinion but when a fish has dropsy its chances are slim anyway.I would only recomend them as a final resort and i hope this fish recovers without them.Good luck with whatever you try and keep us all informed.
I have seen this koi - i took a scrape from this koi, and I am certain it is not dropsy, it was covered in Trichodina, so will have been scraping on whatever it could find, hence the stressed look and raised scales.
